Self Pay Information

Feel like you are having to break into your piggy bank to pay for your health care?  Do you feel like you have no control over what you can do and have done for your health?
Insurance coverage and policies  change frequently and provide increasingly more exclusions for your care.
Premiums and deductibles, as well as co-pays and co-insurance amounts are increasing while your benefits are decreasing.

We feel that YOU should be able to have more control over your health care than your insurance company, so we developed some self pay packages and prices to allow YOU to have more control, while saving you money.

The No Surprises Act - 2022
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

•You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

•Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

•If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

•Make sure to save a copy or picture of your Good Faith Estimate.

General Policies:
We do accept many insurance plans, but with the inception of the Affordable Care Act, many more "carve out" policies were developed.  There are so many of these policies that have numerous exclusions and limitations, and we are not in-network with all of them and we can not know which plan you have within that insurance company.

Even if you have insurance, you are not obligated to use it.  You can "opt-out" of your insurance and pay for your care yourself. The self pay prices are generally lower than the charges that we are required to bill to your insurance.  We will ask you to sign an agreement form to opt out of our office billing your insurance in order to take advantage of the self care pricing. We will also require you to sign a contract for payment at that time.

"Opting out" of your insurance at our office does not preclude you from using your insurance elsewhere.  You can also opt back in and use your insurance at our office at a later date, but once you opt back in, you will be required to use your insurance for the remainder of the calendar year (or your insurance year).

Please note:  Medicare patients are not eligible to opt out of their insurance for services that Medicare covers.

Medical Plans:
Monthly Plan:  Up to 2 visits per month, including in-office labs - $250/month or $2800
Yearly Preventative/Wellness Visit:
    20-65 years old - established patient: $225  - new patient: $250

Lab testing is not included in these charges and labs drawn are sent to Physician's Lab for                                                              processing. You will receive separate billing from them for your charges.
