Patient Policy Summary
The following is a brief overview of our office policies. Full policies are available upon request and are acknowledged when you sign our Consent to Treat form.
Inclement Weather
- In the event of inclement or hazardous weather, our office may close to protect the safety of patients and staff.
- If the office closes, you will be contacted to reschedule your appointment.
- Closure updates may also be posted on the LifeArts Integrated Health Center, PC / LifeArts Medical, LLC Facebook pages.
Appointments and Scheduling
- Appointment times vary based on medical need.
- Arriving more than 7 minutes late will require rescheduling.
- Phone and telehealth visits are considered appointments and will be billed.
Cancellations and No-Shows
- Please provide 24 hours’ notice to cancel or reschedule.
- Missed or late-canceled appointments are subject to a $75 fee.
- Repeated missed appointments may result in dismissal from the practice.
Insurance and Payments
- We submit insurance claims as a courtesy; payment is not guaranteed by insurance.
- You are responsible for all charges not paid by your insurance.
- Copays, deductibles, and coinsurance are due at the time of service.
Billing and Collections
- Balances over 30 days may incur late fees.
- Accounts over 90 days past due may be sent to collections and assessed additional fees.
Minors
- A parent or legal guardian is financially responsible for services provided to minors.
- The adult accompanying the minor is responsible for payment at the visit.
Prescription Refills
- Refill requests should be submitted through your pharmacy.
- Same-day refills are not guaranteed.
- Controlled substances and antibiotics require an appointment.
Labs and Testing
- Lab work is sent to a physician’s laboratory.
- Insurance coverage for labs varies; denied charges are the patient’s responsibility.
Forms and Records
- Forms completed by our office are subject to a $35 administrative fee.
- Medical records requests must be submitted in writing.
Communication
- HIPAA-compliant communication is available through the Healow App and patient portal.
- Messages, calls, or digital communication may be billed when appropriate.
After-Hours Care
After hours and weekends, please seek care at an urgent care or emergency room.

Compounded Medications = Custom-Prepared Prescriptions Not Typically Stocked by Standard Pharmacies
When requesting a refill of your compounded medication, please be prepared to provide the following information to ensure accuracy:
- Medication name
- Pharmacy name
- Quantity needed (milliliters, number of capsules, etc.)
Please note that this policy applies only to compounded medications—custom-prepared prescriptions not typically stocked by standard pharmacies. This includes semaglutide, tirzepatide, compounded naltrexone, amlexanox, and others. If you are unsure whether your medication is compounded, please don’t hesitate to ask when you call.
